The contact owns a 2004 Mitsubishi Endeavor. The contact stated that the vehicle was involved in a roll over crash and none of the front air bags deployed. A police report was filed. The driver suffered minor contusions, cuts on his hands, his head hit the window and as a result he became unconscious. The vehicle was declared destroyed. The manufacturer was made aware of the failure and referred her to NHTSA. The vehicle was not repaired. The failure and current mileage was 116,000.

The contact owns a 2004 Mitsubishi Endeavor. While driving 30 mph and approaching a stop, the contact crashed into a telephone pole and a stop sign. The air bags failed to deploy and the contact was wearing his seat belt at the time. The manufacturer has not yet been notified, but a police report was filed and the insurance company was called. The vehicle was towed to an auto body shop. The contact sustained neck injuries and major abrasions on his right arm, back, and head. The failure and current mileages were unknown.

While sitting at a traffic light and waiting for the light to change vehicle behind hit consumer's vehicle, and consumer's vehicle hit vehicle in front, causing a chain reaction. Driver side air bag did not deploy. Driver sustained back injuries. He went to the hospital the next day.
