This is the third time the heater fan stopped working. Had it fixed twice. The first time the relay switch was replaced. The second time the wire connector was fried and they replaced the connectors. Don't know what the problem is now, taking it in tomorrow for repairs. Checked to see if there was a recall.

Update from Jan 5, 2016: The control unit was loosing ground. Added a new grounding poit to the control unit. Cost me $111.30 for parts, labor, and tax.

Fan works only on 4. 1-3 used to work once in a while

Looks like same problem is common.

Had the same problem with a 1998 Astro van. Either too many amps for the wires and connectors to handle or heater resistor too low.

Colorado LX 3.2L

It has been a pain in the ass, because everytime I start the vehicle. I have to walk around to the passenger side of the vehicle to beat on the heater fan with my hand to get it to work. Hitting a bump in the road sometimes causes it to go off. I have to wait a week to get it fixed and the weather has been in the 20's.

Dealer would not cover. Had two electrical parts replace. One was a short harness to the fan.
