brake pedal goes straight to the floor

I was driving on I-80 East bound on evening of June 5th 2019. The cars in front of me slowed down and I pressed the brake all the way down to the floor. The car slows down but fails to completely stop. I tried to turn to the right to avoid the crash, I only managed to turn my car halfway out of the way and my car still hit the car in front of me. It looks like my car is totaled. I towed my Prius to the Toyota dealership they only provided me with a error code C1259.

I took my Prius into the dealership on May 27, 2016, not yet a year old. I had been having issues with the brake pedal going straight to the floor with no resistance and the skid light coming on. It seems to be happening at speeds around 25 mph. I have noticed it a few times but the latest and scariest was the night of the 26th on my way home from work. I was in traffic and was slowing down to a stop when I pressed on the brakes a little bit harder as the car in front of me stopped, the pedal it went straight to the floor with no resistance and the skid light came on but the car did NOT stop. I had to basically pump the brakes multiple times to get the car to stop. I actually was steering toward the sidewalk as not to rear end the car in front of me. I missed them by inches.

The dealership had the car for a week and could not duplicate the issue. I had told them that in the nine months I'd owned it this had happened maybe five times and I could not duplicate it either. They basically told me I didn't know how to drive. They did call a district service manager to come up to my area and go for a drive with me to see if it was the way I was driving. I'm sorry but when I press on the brake pedal I expect the car to stop. The three gentlemen in the car with me did tell me that they didn't see anything wrong with my driving but they also had no idea what was going on the with car since they could not duplicate it.

Here we go again. Driving to work again todayI had another issue with the brakes. I made it further than a few days ago. I actually made it all the way into work today but when I was pulling up near a co-workers vehicle to back into the spot beside his truck, I pressed on the brakes to slow stop but the brakes went all the way to the floor again. I was driving about 5 or 6 mph. I could not stop, again I had no control of the brakes. I pumped them multiple times and nothing. I was able to steer past his front bumper and missed his truck. I was able to stop only after I jacked the wheel all the way to the left and basically the momentum stopped so the car stopped. This is the second time in three days. This is past dangerous at this point. The dealership will not be happy with me tomorrow when I go in but I will demand they do something about it this time!

While driving to work after a snowstorm I was driving like I always do in the snow (I've been driving in snow for at least 30 winters). I was coming upon a traffic light, that had just turned yellow. I touched my brakes to start to slow down, I was still a long ways from the light, gently touching the brakes everything was fine, no slipping or sliding. As I approached the light I gently increase the brake pressure to slow down more to eventually come to a stop. This is when the brake pedal went completely to the floor with no resistance. The car did not even attempt to slow down or stop. At this point I was getting very close to entering the intersection so I tried to pump the brakes. Again there seemed to be no resistance and the car would NOT stop. I was able to steer the car into a business driveway at the intersection. There I was able to stop in deeper snow. This does not seem to be an anti-lock brake issue. There was nothing when I depressed the pedal harder. Not cool! Very dangerous. This is not the first time or last time this has happened. I will be reporting those instances as well.
