Each problem must be reported as an individual, separate complaint!

Please (PLEASE!!) fill out this form once for each problem that has happened to your vehicle. If the same problem has happened 3 times, report it 3 times. Say your engine light is on and your car is running roughly, report that as two separate complaints.

This way your vehicle complaints will show up accurately in our problem graphs.
It also helps fight poverty, saves endangered species, and you'll win the lottery.1
