headlights do not work and then they do. headlights turn on and off separately and then flicker off. then the lights simply will not turn on

Grand Caravan SXT 4.0L

Headlights work intermittently or not at all. I have replaced the light switch with no improvement. I have also tried disconnecting the battery for a few minutes to reset the TIPM. Sometimes this helps for a day or two then the lights won't work again.

Update from Oct 12, 2015: Replaced the TIPM problem seems to be fixed. Going on two months now.
