The partition between the Engine coolant and Transmission fluid in the radiator failed allowing the two fluids to mix, this also allowed said chemical slurry into the engine, and transmission, respectively. This completely destroyed the engine and transmission. A local shop replaced the busted radiator, and attempted to salvage the engine and transmission, at a cost of more than $3000. Despite their best efforts the truck had several more episodes of engine and transmission trouble (including overheating, and having to flush the transmission fluid several more times $$). Shortly thereafter I was told it would need a new engine, and transmission.
