I've had two of these tires fail due to separation. (in both cases, I purchased the same tire as a replacement for the failed tire.) is there a history of problems with this tire" has there been a recall? should I expect the manufacturer or the dealer to replace all four of these tires with a different brand or model"( dot number: Tire size: P206/70R14 )

PE00020; tire tread separation: While driving at 70 mph, tread on the passenger's side rear tire came off. Driver was able to control vehicle. Three other passegner's were in the vehicle. No one was injured. Tire size P195/75R14. Tires were on a 95, Plymouth, Voyager. Not original tire equipment. Firestone was sent an E-mail about the tire. The vehicle spinned clock wise and counter clock wise and swerved from one lane to another until it stopped in a ditch, causing damage to bumper and muffler.
