Car hesitates to accelerate after stops, and then it lurches.

A few times there is hard shifts. First is backing out of driveway, then placing in drive. The first shift as accelerating is usually hard.

Another time there's hard shifts is when give gas to creep forward, then maybe harder acceleration usually a pretty hard push comes.

Many times when coming to a stop or when getting into garage, the car jumps forward and I have to apply extra pressure on brakes. This is not consistant but it appears often. Initially it was just annoying, but lately, I have been experiencing neck pain when the car jerks. Am very nervous to drive this now as it might hit cars that are standing still at the traffic light.
