The contact owns a 2015 Mazda Mazda6 equipped with Firestone Tires, Tire Line: Firehawk Indy 500, Tire Size: 225/45/R19, DOT Number: EJE8CFJ3819. The contact stated while driving 40 MPH, the front passenger-€™s side tire experienced a blowout. The contact stated that after pulling over, he noticed the wheel was angled inward and had detached from the wheel hub and that there was damage on the sidewall. The dealer was not contacted. The tire was replaced with a spare while awaiting the diagnostic test and repair at independent mechanic. The tire was the original equipment. The manufacturer was notified of the failure. The failure mileage was 88,513.

While driving home from the shore today going 70mph I heard a popping sound and within 15 second my 2015 Mazda 6 13,000 miles was out of control I barley made it crossing four lanes of traffics to get onto the shoulder. Once I exited the car I noticed the left rear tire P245/65R19 sport was the issue upon further examination I noticed the tire broke off approx 1" above where the dot numbers are located. The inside of tire to the rim also broke off completely from the rim. The tire was still completely round but was separated from the rim. Still approx 2" of the rubber was still attached to the both sides of the rim. I took numerous pictures of the tire and rim. I'm now afraid the three remaining tires may still have this type of defect. I was a police officer for the past 35 years and would occasionally travel well over 100mph while performing my duties and not once in those 35 years did I ever experience a tire separating from the rim like today. I really feel that if this happened to an average driver that driver would've completely loss control of the vehicle and could've been seriously injured or death to him or to other drivers.
