Spark plug loosens up and comes out of it's thread due to the thread itself. Causes a misfire. Car sounds slightly more clicky than usual and can throw a code. You might be able to retighten it to solve it, but it may come loose again. If so, you'll have to rethread the spark plug hole in the head itself. Generally cheap if you know how to fix it yourself.
