I'm reading about this same problem/recall from 2010-2011 Prius. Did they make any changes? We just replaced our 3rd set of low beam bulbs in 15mo and one of those new bulbs is bad within 2 weeks. The previous set with a 2 year warranty blew immediately. I'm about ready to sell this annoying thing, but don't want to get sued for selling them a death trap. It's scary driving without lights and it outrages oncoming traffic when you need to use the brights just to get home.

While the car seems to have headlights, they are very dim - too dim to see the road ahead at night. I took the car to Stone Mountain Toyota and spent $203 to 'replace the headlights" but this did not solve the problem. Actual bill was for $307, but they discounted it to compensate me for the two day rental car I had while they fooled around trying to figure out what was wrong. They said car had burned out headlights. I am not convinced there were burned out headlights. I had told them "it was like I had no headlights." They said they had to remove the bumper to install new headlights. That seems absurd. Who designs a car that you have to remove the bumper to replace the headlights? I have had the car back to the dealer again since then, leaving it over night so they could drive it at night. Stone Mountain Toyota says there is nothing wrong with the headlights. Prior to this I had no problem with the brightness of the headlights. But now the car is unsafe to drive at night. I can't sell the car or trade it in with such a serious safety problem. Other "electrical problems" (radio and back up camera) occurred at the same time, reported in separate report.
