The contact owns a 1995 Ford F350. The vehicle has uniroyal Laredo ltl, size LT235/85R16 tires. While driving at an unknown speed, the front driver side tire tread separated and caused the vehicle to veer off the road, down an embankment, and into some trees. The vehicle crashed and flipped over on the driver side. There were two occupants in the vehicle. The contact's wife was seated in the passenger seat and was fatally injured. The contact received minor injuries. A police report was filed. An investigator provided the complaint information on the contact's behalf. The purchase date and dealer information were unknown. The current and failure mileages were 124,055.

While driving about 60 mph on the highway, the driver heard a loud bang coming from the driver's side tire and discovered the tread separated from the tire, the consumer lost control of vehicle, causing a collision with a concrete barrier. There were no fatalities. Firestone, steeltex at, P265/75R16.

PE 00-040/tread separation/tire failure: Left front tire expereinced a sidewall tread separation while traveling at 45 mph on a 1995, Ford, F350; original equipment, tire size lt 215/85R16, dot# vn0P1wv165. Firestone was not notified. Consumer has since notified Firestone as of 9-5-2001 and was informed by Firestone that there are no recalls, consumer has replaced the tires.

While driving vehicle, 3 out of 6 tires-left front/left rear inside, and right rear inside lost treads at various times. Was on cruise control at 50mph, and at other times was doing 25mph. Had tires checked, nothing was found that caused tread to peel off. Firestone/Bridgestone 273swp; LT21585416-E; with 11,000 miles. Tires on 1995, Ford F350, crew cab. Bridgestone/Firestone refused to replace tires.

Tire tread came off veh and hit veh damaging it. Insd was able to control the veh and did not hit anything. Need to sub Firestone tire is a 235 85 R 16 Firestone R4S. (claim# 59-V549-291)

PE00040, original equipment on 1995, F-350, tire size 21585R16. Inside rear tire tread came off, almost causing a crash. A previous tire at approximately 20,000 did the same as this one being reported. (consumer indicated 2 fatalities on the report, however no further information was provided).

Following servicing and installation of new exhaust manifold, had a bolt/bracket come loose while towing a horse trailer, bolt/bracket impacted left rear tire, inducing a blow out through the tread. Failure to properly install bolt/bracket is the primary cause of this damage. Extensive damage from this induced tire failure on under body of truck in front of and behind the left rear truck wheel as well as underbody dents to aluminuim horse trailer.
