The vehicle was taken to Carmax where the headlamp was replaced however, the front driver's side headlamp failed. No warning lights illuminated. The vehicle was not diagnosed or repaired. The dealer was not notified of the failure. The failure mileage was approximately 80,000.
Headlights causing moving shadows, blind areas, phantom objects. Unsafe because visibility is reduced, compromised. Dangerous to operators and passengers. Has a high probability of causing an accident.
