Vehicle was rear-ended on November 4, 2022. The driver was wearing a seatbelt but the driver airbag did not deploy after being rear-ended. After the accident, the dashboard airbag indicator warning light turned on. The vehicle did not show any airbag issues or warnings prior to the accident. The emergency parking brake does not properly engage after the accident.

I noticed that my car would not brake properly if I happened to be breaking over an uneven service years ago. So, I drive to avoid having to do so. But, then recently it started making a squealing noise along with breaking. It comes and goes. Then, a clunking noise started when reversing. Now, the clunking noise comes in either direction. The squealing has graduated to scratching and scraping when I accelerate or decelerate. It comes from my front, passenger side tire and when I turn my steering wheel all the way to the right, it gets very loose - whereas turning all the way to the left still feels appropriately tight. I think the looseness is connected to the front right passenger side wheel, as if a piece has broken or is loose - because it comes and goes. This all happens when I'm moving, no matter what terrain it is (highway, city street, etc.) and whether I am turning or not.

The contact owns a 2009 Toyota Yaris. While depressing the brake pedal at 35 mph, the vehicle failed to respond and the ABS warning light illuminated. As a result, the contact crashed into another vehicle. The air bags failed to deploy. A police report was filed. The contact sustained a concussion and upper back pain that required medical attention. The vehicle was towed to an independent mechanic, but was not diagnosed; however, the body damage was repaired. In addition, the contact received notification of NHTSA campaign numbers: 14V168000 (air bags) and 14V169000 (seats). The parts to do the repairs were unavailable. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repairs. The manufacturer was notified of the failure. The failure mileage was 71,000. Parts distribution disconnect.

I braked to avoid hitting an animal which emerged from the ditch. The car fishtailed, lost control and flipped once landing on it's top. The driver's side was damaged and the insurance company totaled the car. I did hit my head against the driver's side window, but the blow was not severe. My concern is that the side airbag did not deploy. The anti-lock brakes did not work either and have only worked intermittently for quite some time.

I was decelerating on a downhill shortened exit ramp, going from 70 mph to a stop within approximately 500 feet. When I applied the brakes, it felt as if the brakes were "metal to metal" and the vehicle barely stopped before hitting the vehicle in front of me. Since that time, the same type of incident has occurred 3 more times, always when decelerating rapidly. (when decelerating at a slow rate of speed, they work fine.) I have had the brakes checked, but they said they were "normal". they certainly are not!
