New Chevy volt, bought with 2000 miles. Brakes failed in very slow stop-and-go downhill traffic on a approximately 15 degree grade, after riding the brakes on and off for about 20 minutes on 85-90 degree day. Pressing the brakes fully twice did not work and the vehicle rolled about 10-15 feet into the car in front doing damage to both vehicles. No traction whatsoever. This problem occurred once, and subsequent inspections by GM's and 2nd dealer did not reproduce the problem. Similar transient brake-by-wire failure reports on the 2017 Volt can be found on this site.

Update from Sep 12, 2018: The inspectors found no error codes, but this may mean that it is a design flaw that is just hard to track as it occurred after 20-25 minutes of continuous downhill braking in stopped traffic. Occurred 9 August 2018 approximately 3:20 PM.
