  1. 2003 Mazda MAZDA6 AIR BAGS:PASSENGER SIDE FRONTAL, AIR BAGS:PASSENGER SIDE FRONTAL NHTSA Campaign #16V354000

    • Date Announced: MAY 25, 2016
    • Vehicles Affected: 448,320

    Summary: Mazda North American Operations (Mazda) is recalling certain model year 2004-2011 RX-8, 2004-2006 MPV, 2003-2008 Mazda6, and 2006-2007 Mazdaspeed6 vehicles, originally sold, or ever registered, in Alabama, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, South Carolina, Texas, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands, or "Zone A." Additionally, unless included in "Zone A" above, Mazda is recalling certain model year 2004-2008 RX-8, 2004-2006 MPV, 2003-2008 Mazda6 vehicles, originally sold, or ever registered, in Arizona, Arkansas, Delaware, District of Columbia, Illinois, Indiana, Kansas, Kentucky, Maryland, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, Tennessee, Virginia, and West Virginia, or "Zone B." Lastly, unless included in "Zone A" or "Zone B" above, Mazda is recalling certain model year 2004 Mazda RX-8 and MPV, 2003-2004 Mazda6 vehicles originally sold, or registered, in Alaska, Colorado, Connecticut, Idaho, Iowa, Maine, Massachusetts, Michigan, Minnesota, Montana, New Hampshire, New York, North Dakota, Oregon, Rhode Island, South Dakota, Utah, Vermont, Washington, Wisconsin, and Wyoming. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules, and used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the front air bags, these inflators may rupture due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.

    Consequence: An inflator rupture may result in metal fragments striking the vehicle occupants resulting in serious injury or death.

    Actions: Mazda will notify owners, and dealers will replace the passenger frontal air bag inflator, free of charge.   2. 2003 Mazda MAZDA6 AIR BAGS:PASSENGER SIDE FRONTAL NHTSA Campaign #15V869000

    • Date Announced: DECEMBER 23, 2015
    • Vehicles Affected: 43,870

    Summary: Mazda North American Operations (Mazda) is recalling certain model year 2003-2008 Mazda6 vehicles manufactured May 29, 2002, to May 5, 2008, 2006-2007 Mazdaspeed6 vehicles manufactured July 1, 2005, to June 29, 2007, and a limited production of model year 2004 RX-8 vehicles manufactured June 25, 2003, to June 30, 2003. The inflators in the front passenger air bags are susceptible to rupture in the event of a crash necessitating the deployment of the front passenger air bag.

    Consequence: In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

    Actions: Mazda will notify owners, and dealers will replace the passenger's frontal air bag with a new one, free of charge. The recall began on February 16, 2016.   3. 2003 Mazda MAZDA6 AIR BAGS:FRONTAL:DRIVER SIDE INFLATOR MODULE NHTSA Campaign #15V382000

    • Date Announced: JUNE 17, 2015
    • Vehicles Affected: 442,266

    Summary: Mazda North American Operations (Mazda) is recalling certain model year 2003-2008 Mazda6 vehicles manufactured May 29, 2002, to May 5, 2008, 2004-2008 RX-8 vehicles manufactured April 10, 2003, to February 18, 2008, and 2006-2007 Mazdaspeed6 vehicles manufactured August 4, 2005, to June 29, 2007. The affected vehicles are equipped with a dual-stage driver frontal air bag that may be susceptible to moisture intrusion and other factors, including manufacturing variability that, over time, could cause the inflator to rupture.

    Consequence: In the event of a crash necessitating deployment of the driver's frontal air bag, the inflator could rupture with metal fragments striking the driver or other occupants resulting in serious injury or death.

    Actions: Mazda will notify owners, and dealers will replace the driver's frontal air bag inflator, free of charge. The recall began on November 20, 2015.   4. 2003 Mazda MAZDA6 AIR BAGS NHTSA Campaign #14V773000

    • Date Announced: DECEMBER 04, 2014
    • Vehicles Affected: 5,070

    Summary: Mazda North American Operations (Mazda) is recalling certain model year 2003-2006 Mazda6, and model year 2004-2005 RX-8, and MPV vehicles originally sold, or ever registered, in geographic locations associated with high absolute humidity. Specifically, vehicles sold, or ever registered, in Alabama, Florida, Georgia, Hawaii, Louisiana, Mississippi, Texas, Puerto Rico, Saipan and Guam. Upon deployment of the passenger side frontal air bag, excessive internal pressure may cause the inflator to rupture.

    Consequence: In the event of a crash necessitating deployment of the passenger¿s frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

    Actions: Mazda will notify owners, and dealers will replace the front passenger air bag inflators, free of charge. The recall began February 4, 2015.   5. 2003 Mazda MAZDA6 AIR BAGS NHTSA Campaign #14V362000

    • Date Announced: JUNE 23, 2014
    • Vehicles Affected: 19,544

    Summary: Mazda North America Operations (Mazda) is recalling certain model year 2003-2004 Mazda6 vehicles and 2004 RX-8 vehicles to address a safety defect in the passenger side frontal air bag which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag.

    Consequence: In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking and potentially seriously injuring the passenger seat occupant or other occupants.

    Actions: Owners of all vehicles on which a defective air bag may have been installed as original equipment will be notified. Dealers will replace the defective air bag inflator, free of charge.     Summary: Mazda is recalling certain model year 2003-2004 Mazda6 vehicles manufactured January 14, 2003, through May 29, 2003; and 2004 RX-8 vehicles manufactured June 25, 2003, through June 30, 2003, to address a safety defect in the passenger side air bag which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag.

    Consequence: In the event of a crash necessitating deployment of the passenger's air bag, the inflator could rupture with metal fragments striking and potentially seriously injuring the passenger seat occupant or other occupants.

    Actions: Owners of all vehicles on which a defective air bag may have been installed as original equipment will be notified.   7. 2003 Mazda MAZDA6 EXTERIOR LIGHTING:HEADLIGHTS NHTSA Campaign #09E025000

    • Date Announced: MAY 11, 2009
    • Vehicles Affected: 42,540

    Summary: Dope, Inc. Is recalling 42,540 combination corner and bumper lamp assemblies of various part numbers sold for use as aftermarket equipment for various passenger vehicles. These headlamps fail to conform to the requirements of Federal Motor Vehicle Safety Standard no. 108, "lamps, reflective devices, and associated equipment." These lamps do not contain the required amber side reflectors.

    Consequence: Decreased lighting visibility may result in a vehicle crash.

  8. 2003 Mazda MAZDA6 EXTERIOR LIGHTING NHTSA Campaign #09E012000

    • Date Announced: APRIL 07, 2009
    • Vehicles Affected: 16,270

    Summary: Sabersport is recalling 16,270 combination corner and bumper lamp assemblies of various part numbers sold for use as aftermarket equipment for various passenger vehicles. These headlamps fail to conform to the requirements of Federal Motor Vehicle Safety Standard no. 108, "lamps, reflective devices, and associated equipment." These lamps do not contain the required amber side reflectors.

    Consequence: Decreased lighting visibility may result in a vehicle crash.

  9. 2003 Mazda MAZDA6 EQUIPMENT NHTSA Campaign #09E011000

    • Date Announced: APRIL 02, 2009
    • Vehicles Affected: 2,323

    Summary: Thule, Inc. Is recalling 2,323 owner-installed roof-mounted fit kits, models 3068 and 3069, manufactured between January 1, 2008 and February 28, 2009 that are used with thule podium foot packs, models 460 and 460R. These fit kits were sold as aftermarket equipment for use on the vehicles listed above. The bolt(s) used to secure the foot to the vehicle was manufactured to a hardness that may cause the bolt(s) to be brittle and potentially break under stress.

    Consequence: This may result in the bolt(s) snapping off during use or without a load. The unsecured rack and load can then come loose while driving creating a potential road hazard, possibly resulting in a vehicle crash.

    Actions: Thule will mail a retro fit kit along with repair instructions to owners free of charge. The safety recall is expected to begin on or before April 13, 2009.   10. 2003 Mazda MAZDA6 FUEL SYSTEM, GASOLINE:DELIVERY NHTSA Campaign #03V531000

    • Date Announced: DECEMBER 16, 2003
    • Vehicles Affected: 56,700

    Summary: On certain passenger vehicles, the fuel sender unit (fsu) may have been improperly installed in the fuel tank, producing an inadequate seal, and fuel may leak.

    Consequence: A fire could occur in the presence of an ignition source.

  11. 2003 Mazda MAZDA6 EXTERIOR LIGHTING:FOG LIGHTS NHTSA Campaign #03V206000

    • Date Announced: JUNE 05, 2003
    • Vehicles Affected: 64,000

    Summary: On certain mini vans and passenger vehicles, the socket holder in the fog lights is installed in the lamp casing using pressure. During operation of the fog light, the heat from the bulb can expand the socket holder. The expansion can force the socket holder rearward. After repeatedly being subject to this expansion, if the fog light is also subjected to vibration during vehicle operation, the socket holder with the bulb holder and the wiring harness may separate from the fog light casing.

    Consequence: Should this occur, the bulb socket and the harness can drop inside the bumper and possibly but rarely, the heat generated by the bulb can cause the bumper to burn.

    Actions: Dealers will install a spring to prevent the socket holder from separating from the lamp case. Owner notification began on July 14, 2003.   12. 2003 Mazda MAZDA6 PARKING BRAKE:INDICATOR LIGHT NHTSA Campaign #03V207000

    • Date Announced: JUNE 05, 2003
    • Vehicles Affected: 45,600

    Summary: Certain passenger vehicles fail to comply with requirements of Federal Motor Vehicle Safety Standard no. 135, "passenger car brake systems." During production, the float in the fluid level sensor in the brake reservoir can be forced off of the guide rail during the filling of the master cylinder.

    Consequence: Should this occur, it is possible that the float will not return to the guide rail and the brake fluid level warning indicator may not function.

    Actions: Mazda has not yet provided a remedy for this campaign. Owner notification began July 14, 2003.
