  1. 2004 Mazda RX-8 FUEL SYSTEM, GASOLINE:DELIVERY:FUEL PUMP NHTSA Campaign #16V793000

    • Date Announced: OCTOBER 31, 2016
    • Vehicles Affected: 69,447

    Summary: Mazda North America Operations (Mazda) is recalling certain model year 2004-2008 RX-8 vehicles manufactured April 10, 2003, to February 18, 2008. The affected vehicles have fuel pump sealing rings that may deteriorate due to exposure to heat from the engine or exhaust pipe.

    Consequence: The deteriorated seals may leak fuel, which, in the presence of an ignition source, can increase the risk of a fire.

    Actions: Mazda will notify owners, and dealers will replace the fuel pump sealing ring set and add a thermal insulation pad to the fuel tank, free of charge. Parts are not currently available. Mazda will notify owners about the issue and will second a second notice when remedy parts are available. The manufacturer did not provide a schedule for these notifications.     Summary: Mazda North American Operations (Mazda) is recalling certain model year 2004-2011 RX-8, 2004-2006 MPV, 2003-2008 Mazda6, and 2006-2007 Mazdaspeed6 vehicles, originally sold, or ever registered, in Alabama, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, South Carolina, Texas, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands, or "Zone A." Additionally, unless included in "Zone A" above, Mazda is recalling certain model year 2004-2008 RX-8, 2004-2006 MPV, 2003-2008 Mazda6 vehicles, originally sold, or ever registered, in Arizona, Arkansas, Delaware, District of Columbia, Illinois, Indiana, Kansas, Kentucky, Maryland, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, Tennessee, Virginia, and West Virginia, or "Zone B." Lastly, unless included in "Zone A" or "Zone B" above, Mazda is recalling certain model year 2004 Mazda RX-8 and MPV, 2003-2004 Mazda6 vehicles originally sold, or registered, in Alaska, Colorado, Connecticut, Idaho, Iowa, Maine, Massachusetts, Michigan, Minnesota, Montana, New Hampshire, New York, North Dakota, Oregon, Rhode Island, South Dakota, Utah, Vermont, Washington, Wisconsin, and Wyoming. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules, and used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the front air bags, these inflators may rupture due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.

    Consequence: An inflator rupture may result in metal fragments striking the vehicle occupants resulting in serious injury or death.

    Actions: Mazda will notify owners, and dealers will replace the passenger frontal air bag inflator, free of charge.   3. 2004 Mazda RX-8 AIR BAGS:PASSENGER SIDE FRONTAL NHTSA Campaign #15V869000

    • Date Announced: DECEMBER 23, 2015
    • Vehicles Affected: 43,870

    Summary: Mazda North American Operations (Mazda) is recalling certain model year 2003-2008 Mazda6 vehicles manufactured May 29, 2002, to May 5, 2008, 2006-2007 Mazdaspeed6 vehicles manufactured July 1, 2005, to June 29, 2007, and a limited production of model year 2004 RX-8 vehicles manufactured June 25, 2003, to June 30, 2003. The inflators in the front passenger air bags are susceptible to rupture in the event of a crash necessitating the deployment of the front passenger air bag.

    Consequence: In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

    Actions: Mazda will notify owners, and dealers will replace the passenger's frontal air bag with a new one, free of charge. The recall began on February 16, 2016.   4. 2004 Mazda RX-8 AIR BAGS:FRONTAL:DRIVER SIDE INFLATOR MODULE NHTSA Campaign #15V382000

    Summary: Mazda North American Operations (Mazda) is recalling certain model year 2003-2008 Mazda6 vehicles manufactured May 29, 2002, to May 5, 2008, 2004-2008 RX-8 vehicles manufactured April 10, 2003, to February 18, 2008, and 2006-2007 Mazdaspeed6 vehicles manufactured August 4, 2005, to June 29, 2007. The affected vehicles are equipped with a dual-stage driver frontal air bag that may be susceptible to moisture intrusion and other factors, including manufacturing variability that, over time, could cause the inflator to rupture.

    Consequence: In the event of a crash necessitating deployment of the driver's frontal air bag, the inflator could rupture with metal fragments striking the driver or other occupants resulting in serious injury or death.

    Actions: Mazda will notify owners, and dealers will replace the driver's frontal air bag inflator, free of charge. The recall began on November 20, 2015.   5. 2004 Mazda RX-8 AIR BAGS NHTSA Campaign #14V773000

    Summary: Mazda North American Operations (Mazda) is recalling certain model year 2003-2006 Mazda6, and model year 2004-2005 RX-8, and MPV vehicles originally sold, or ever registered, in geographic locations associated with high absolute humidity. Specifically, vehicles sold, or ever registered, in Alabama, Florida, Georgia, Hawaii, Louisiana, Mississippi, Texas, Puerto Rico, Saipan and Guam. Upon deployment of the passenger side frontal air bag, excessive internal pressure may cause the inflator to rupture.

    Consequence: In the event of a crash necessitating deployment of the passenger¿s frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

    Actions: Mazda will notify owners, and dealers will replace the front passenger air bag inflators, free of charge. The recall began February 4, 2015.   6. 2004 Mazda RX-8 AIR BAGS NHTSA Campaign #14V362000

    • Date Announced: JUNE 23, 2014
    • Vehicles Affected: 19,544

    Summary: Mazda North America Operations (Mazda) is recalling certain model year 2003-2004 Mazda6 vehicles and 2004 RX-8 vehicles to address a safety defect in the passenger side frontal air bag which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag.

    Consequence: In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking and potentially seriously injuring the passenger seat occupant or other occupants.

    Actions: Owners of all vehicles on which a defective air bag may have been installed as original equipment will be notified. Dealers will replace the defective air bag inflator, free of charge.     Summary: Mazda is recalling certain model year 2003-2004 Mazda6 vehicles manufactured January 14, 2003, through May 29, 2003; and 2004 RX-8 vehicles manufactured June 25, 2003, through June 30, 2003, to address a safety defect in the passenger side air bag which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag.

    Consequence: In the event of a crash necessitating deployment of the passenger's air bag, the inflator could rupture with metal fragments striking and potentially seriously injuring the passenger seat occupant or other occupants.

    Actions: Owners of all vehicles on which a defective air bag may have been installed as original equipment will be notified. Dealers will inspect vehicles, and where a defective air bag inflator is identified, the inflator will be replaced, free of charge. The safety recall began on June 3, 2013.   8. 2004 Mazda RX-8 SUSPENSION:FRONT:CONTROL ARM:LOWER BALL JOINT NHTSA Campaign #05V325000

    • Date Announced: JULY 20, 2005
    • Vehicles Affected: 27,800

    Summary: On certain passenger vehicles, due to improper forging of the lower control arm, cracks may occur in the ball joint socket.

    Consequence: In severe driving conditions, the ball may separate from the ball joint socket and a loss of steering may occur, which could result in a crash.

  9. 2004 Mazda RX-8 ENGINE AND ENGINE COOLING NHTSA Campaign #05V317000

    • Date Announced: JULY 07, 2005
    • Vehicles Affected: 52,000

    Summary: If certain passenger vehicles are parked and the engine is operated at high RPM's for an excessive length of time, some of the parts around the exhaust system can melt and produce a variety of malfunctions. Problems caused by the excessive heat build-up can range from inoperative oxygen sensor, neutral switch and back up lights, problems with the parking brakes, malfunctions of the gas gauge and/or possible fuel leaks resulting from heat damage to the fuel tank.

    Consequence: Fuel leakage, in the presence of an ignition source, could result in a fire.

  10. 2004 Mazda RX-8 AIR BAGS:FRONTAL NHTSA Campaign #04V074000

    • Date Announced: FEBRUARY 17, 2004
    • Vehicles Affected: 29,300

    Summary: As a result of an NCAP test, certain passenger vehicles are being recalled for improperly wired front passenger seat air bags.

    Consequence: The air bag will not provide adequate protection when the vehicle is involved in a frontal crash.

  11. 2004 Mazda RX-8 POWER TRAIN:MANUAL TRANSMISSION NHTSA Campaign #04V075000

    • Date Announced: FEBRUARY 17, 2004
    • Vehicles Affected: 12,000

    Summary: On certain passenger vehicles with manual transmissions, the heat insulator may be cracked causing abnormal noise.

    Consequence: Continued operation of the vehicle can cause the insulator to come off and could pose a hazard to following vehicles.
