The car's lithium batteries caught fire to the point of smoldering. This happened at the end of the charge cycle. The car was parked in our garage and if the fire department did not come to help, the fire could have intensified and burned down our whole house. We are very thankful we caught it so quickly. No warnings or issues were apparent with the vehicle when it was parked and put on the charger.

What component or system failed or malfunctioned, and is it available for inspection upon request" - Internal charging system or vehicle battery pack malfunctioned & caught fire. Vehicle is available for inspection now, but not sure about when it is removed from our property. How was your safety or the safety of others put at risk? - It was a big fire! Close to structures, vehicles & dry foliage -?" super lucky it didn't spread. Smoke was very noxious and affected our entire neighborhood. Neighbors have reported respiratory irritation since the fire. I now have a cough, burning throat and headaches. Has the problem been reproduced or confirmed by a dealer or independent service center? - There's a burned out van at the end of my driveway... specific instance of this problem does not need confirmation Similar issues are reported on the internet. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? - Not yet. Chrysler says it'll take 2-3 days for issue to be "escalated". Insurance is sending someone. Fire department HazMat crew verified that the battery pack is now "cold" but toxic chemicals are a public safety hazard. Oakland Fire incident #91242. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear" - July 2022 the vehicle refused to charge & displayed a "turtle" icon. Took it to the dealer and told of the charging/fire recall. Stopped charging until all the recall parts were available which wasn't until August 2023. After repair, we were told the car was OK & it was safe to resume charging the car. Other details: - The vehicle caught fire while charging, engulfed in flames, had to be extinguished by Fire department, total loss. Car plugged in at ~4:30pm and we heard a "pop" at ~8:20pm. So the battery had reached 100%. Ran out and there was small fire dripping from middle underside of the car-?" quickly spread & consumed the entire car

Blown head gasket 2017 chrysler pacifica 82000 miles. Spark plugs replaced. All are black with heavy carbon buildup. Check engine light went on as did the engine stop start warning. While driving my vehicle, it suddenly stalled and told me to put it in park. I was driving approximately 25 miles per hour. I had to pull over and put it in park. It then allowed me to restart. There were no warning lamps and happened spontaneously. I have not had the issue reproduced but this wad part of a recall for this vehicle in 2018 that seems to have returned.

The van began to run rough in the mornings when I first started up. I initially thought a tire was flat. But all tires were fine. It did this several mornings but then would begin driving smoothly as normal about 2-3 minutes after the drive started. A little less than two weeks later the check engine light came on for the first time. It ran roughly but lasted a little longer; maybe 5 minutes. The check engine light would go off. It did this a couple of times. The third time, the check engine light came on and started blinking. Then maybe 20 minutes into the drive the van began to overheat. I turned on the heat and the gauge went down to normal right away and drove as usual. The check engine light stopped blinking but stayed on. No problems for the rest of the hour-long drive. The next day, the same thing happened while i was on the highway. It started overheating, but the overheating lasted longer and intensified at higher speeds, so I had to drive about 24-35 on an expressway. I took it to a Fiat Chrysler Auto/Mopar Service repair shop immediately. The manager suspected that there was a crack in the cylinder and coolant was leaking into possibly the engine or somewhere else. They said they've seen this same crack, in the same location on many other Chrysler Pacificas. They took pictures (see attached) and sent it to FCA/Mopar. It took several weeks for an FCA/Mopar Inspector to come out. But once they did, they confirmed a crack and the van required a new engine. I'm told this engine is on backorder and it will take months before I can get a new one. I've read many stories online of other people having similar issues. The fact that this engine is on backorder for months is telling. The company Chrysler Customer Care/Mopar/Stellantis has not been helpful in providing useful timelines throughout this repair process. In addition to the above issue, I've also had to replace both main (2x) & aux batteries and experienced problems with sliding doors failing to open.

Car was serviced for a gas leak at a Chrysler Dealership the previous day. The next morning, I was driving at a low speed (<20 mph) when an error message appeared "Car must be in P (park) to place in drive." I switched to P and tried to restart the engine. It turned over several times but never restarted. A police officer had to manually disengage the transmission and emergency brake to put the car in neutral and roll it to a turn lane (rather than in heavy traffic). The car was towed to a dealership and is still being diagnosed. We had no driving issues prior to the car stalling this morning, even with the previous gas leak mentioned above.

Top portion of fuel filler neck broke off vehicle during normal refueling process. Top of capless system fractured from filler neck and came off with gas pump nozzle as it was removed from vehicle. Capless fuel system no longer has a seal mechanism to contain fuel vapors. Fuel vapors will potentially leak from vehicle during normal driving as well as parked conditions. This leakage will worsen as weather begins to warm up. Potential to trigger OBDII malfunction when vehicle runs diagnostics for leakage.

While driving our minivan it stalls or it seems like the gas isn-€™t getting to the engine. It has happened when I-€™ve been accelerating from a side road across a very busy road on more thank e occasion. We thought it was the gas we were using. Now we-€™ve been advised it is due to a -€œcrank shaft sensor-€. (?) We may not be eligible to get it fixed because we got our 2017 Pacifica in Oct 2016, drove it approx. 15,000 miles/ year so we may not be able to get it fixed for free.

I have had engine problems for over a year now. My car has had constant issues with rough idling and also jerking while driving. The jerking has been fixed for now but the rough idling has became worse to the point where the car feels as if it will stall out. My car has been at the dealership for repair since 12.22.22 and is still there today 1.11.23 - I was told by another repair shop (while getting brakes and an oil change ) that my car had 3 holes in the crankshaft and that was a problem because there should only be 2 holes. The dealership has replaced an EGR and now is talking about something on the engine to replace (but not the whole engine) and they have still not looked at the crankshaft to see if there are the incorrect number of holes. This is my primary vehicle and no one can seem to fix the problem

We had the FCA Recall #Z11, NHTSA Recall #22V-077 repaired on this vehicle. One week later, the plug-in hybrid Pacifica was parked in the garage, plugged in but done charging. In the middle of the night, the battery spontaneously combusted. We heard a loud boom sound and found the car smoldering and emitting significant amounts of smoke. The car then burst into flames. The fire department soaked the car with water but it continued to reignite for approximately two hours despite the fire department-€™s best efforts to extinguish the fire. The car was completely destroyed. It also caused significant chemical smoke damage to our garage and home. As soon as we found it smoking, we rolled it out of the garage, which saved our house from catching on fire. Luckily our children were not in or next to the car at the time the battery combusted. There were no warning signs in the car prior to this incident. The vehicle has been inspected by insurance and is available for further inspection. At least two members of the family experienced adverse health consequences following the smoke exposure.

The vehicle stalled on the road in the middle of an intersection and then again in a turn lane a few hours later in the same day. The car was in drive, rumbled near the front of the vehicle, lurched forward, and then the engine cut out and would not restart. Trying to start the car again made a REVVVVVV-putta-putta-putta noise and there was not any control of the ignition on my end. It revved up and then hovered around 1/2 of 1 RPM on the dash then it would cut out and ask me to put it in park and start again. Hitting the engine start button did nothing and the dash electronics seemed to cut in and out. Each time I went to restart the car, it went from being in drive to stalling to needing 5+ minutes before it could be driven again, this happened without warning. My family and myself were in the car when we were first stuck in the middle of an intersection. If traffic had been heavier there would have been no warning and I would just be stuck with no way to move the car out of the way quickly. AND with electric systems malfunctioning I'm not confident that the airbags would deploy effectively in the event of a crash should we be left stranded at another intersection after the car stalls. I drove the car to the dealership and on my way a very similar incident occurred, i used my phone to record the incident this time and while the car started right up in the dealer's driveway, I was able to say "NO it literally stalled on my way here, look at this video" The video is just 5 minutes of me sitting at another intersection with people honking and cursing at me as they drive by. The car starts up, revved up then dies immediately. Earlier, the AUTOSTART light came up, but the second time the CHECK ENGINE light was on and off again. The vehicle is at the dealership now and we are hoping it will be repaired, but (looking over other complaints here) I'm very worried about the safety of my family and other drivers with these vehicles on the road.

Driving down the road and suddenly engine shut off without warning, lost power immediately, RPMs went to zero, dash said "Shift to Park." Tried to pull over, didn't get completely out of the way because no power from engine so car had to swerve around us. Shifted to park, tried to restart, didn't work until third time to restart car so that I could drive home. Ridiculously unsafe for the car engine to just shut off without warning in middle of road. Looks like this is the same recall Chrysler tried to fix years ago. Someone is going to die from this defect. Thank God I was on a local road and not the highway when this happened. This car model should be taken off the road immediately.

While traveling on a dry highway, with good tires/new tires, and decelerating from 45 mph and approaching a red light, and a moderate, but firm pressure on the brake to bring the van to a full stop, the speed decreased to 15 mph, with the tachometer stable at 800 rpm and then to 10 mph, with both hands on the wheel, left foot on the dead pedal, and right foot on the middle of the brake pedal. As the speedometer made the transition at 10 MPH, the tachometer surged to 1500 RPM and then to 3000 in under 3 seconds, which cause me to initiate an emergency action drill of stabbing the brake pedal hard three times and rotating the transmission control to the lowest gear. The transmission refused to shift lower, and the control of the vehicle turned on oa wrestling match and the tachometer surged from 1500 RPM, them 3000 RPM, then 4500 RPM, and then 6000 RPM, each time the vehicle acting like it was repeatedly being rear-ended at a speed exceeding 15 MPH. The engine could not be disengaged and aceelaretd uncontrollably to over 40 MPH, through a busy intersection, and required over 24 hard break stages over a 10 second period to actually stop the car, and the engine refused to disengage until stopped. Once the car was restarted it immediately surged forward trying to fight through the hard applied brakes. The cruise control was never on the entire time. I then stut down the engine once I was able to wretle it to a stop with the engine climbing to over 4500 RPM. I then took this vehicle to a safe area, and repeated the conditions, and again as the vehilce speed dropped below 15 MPH, and certainly below 10 MPH, the engine tachometer increased in steps of 1500 MPH, 3000, 4500, 6000 RPM, and no control could be achieved other then violently "stabbing" the brake pedal. Under controlled conditions the behavior was repeatable over 20 times. This is a life threatenign defect, and it appears to be software in origin. I have CAN data streams from the events (23) and video.

Arrigo chrylser dealership in fort pierce Florida stated they repaired issue with computer reset or reprogramming but did not resolve. Continues to stall and lurch and lose power.

we smelled gas on the passenger side and noticed there wasn't a ton of power being given to the engine. Once we got to our destination the gas smell was stronger. We arrived back at our house and the gas smell was really bad at this point and noticed there was gas coming over the headlights. We popped the hood and there was gas everywhere. We got very lucky and the van did not catch on fire. The fuel supply tube (68210442AA) was shooting gas out from a hole in it and it would land a good 20-ft outside the vehicle. This could have killed myself and my family. We went to find the part and come to find out chrysler "redesigned" the part but there is no recall on this part yet.

This vehicle is a handicap conversion van modified by revability / vmi in the modification - revability has replaced the factory fuel tank with one of their own design and construction. In doing so - they use the Chrysler factory components such as the fuel pump, fuel gauge sender unit, emissions connections, etc. However - the fuel gauge has never worked properly since I purchased the van and it gives misleading information about the fuel level. It registers only 3/4 full on the dash when in fact the tank is full. If you try to add any more fuel (as I found out when I first purchased the van) - it affects the emissions controls severely and makes the van un-drivable; in fact the engine will stall unexpectedly in traffic which is a severe hazard. Further - the low tank level is unpredictable so you are never sure when to refill, if the fuel gets too low then the engine can also stop in traffic. The company is well aware of this problem but cannot fix it. I have had it to the original selling dealership (jay hatfield mobility in lee's summit Missouri) several times as revability said the repairs were covered under their manufactures warranty. The dealership has been unable to fix the problem and has since not renewed their contract with revability. So the problem still exists and the powertrain 'check engine' light comes on periodically - which jay hatfield mobility said they had nothing to do with and would not do any further repairs. My wife is handicapped and in daily rehab treatment - so I need this van every day and need it fixed properly - however the van conversion manufacturer has no instructions or information that they can supply for a correct repair. I believe that revability / vmi should be responsible for fixing this problem properly or replacing this van as it is / can be a safety hazard to operate without a properly functioning fuel gauge.
