stock lug bolts and nuts failing

Explorer was brand new from dealership. Drive from Virginia to Florida. Rear wheel came off and we crashed. Lugs and lug nuts failed. It was fixed.

Arrived in Florida, and a few months later, lug nut come apart (top portion). Fixed by dealership and noted in my file.

Fast forward another 5 weeks, another lug nut came apart while in the lug and the wheel, same as others listed above.

Bought torque wrench and proper socket to keep in vehicle and constantly check lug nuts.

First time shame on a Ford, second and third time shame on me. Lug nuts replaced with aftermarket.
